PROCESS FLOW

 

I.  APPLICATION FOR ASSESSMENT (New Applicant/Assessee)

Provide information and process applications for assessment

· Application of assessment shall be filed at the Accredited Assessment Centers.

· The applications may be received by the PO to facilitate the application for assessment. The application shall be forwarded to Accredited Assessment Centers. 

Payment of Assessment Fee shall be made in the ACs.

Processing Officer

Provides information on Qualifications for assessment

· Self-Assessment Guide (TESDA-SOP-CO-07-F22)

· Application Form (TESDA-SOP-CO-07-F21)

· Checklist of Requirements relevant to the qualification arrangements

· Assessment and Certification arrangements.

 

Checks the following:

· Accomplished Self-Assessment Guide

· Application Form properly and completely filled-out

· Submit pictures: 3 pcs., colored, passport size, (3.5cm x 4.5cm with head size ranging from 27 mm to 31mm; white background, with collar; and name printed at the back)

 

Each applicant for Competency Assessment shall be given a reference number which shall be used in all the following assessment documents:

· Application Form

· Self-Assessment Guide (SAG)

· Admission Slip

· Rating Sheet

· Competency Assessment Results Summary (CARS)

· Request applicant to pay the approved Assessment Fee after receiving the complete documents.

· Detached Admission Slip from the Application Form and returns it to the applicant and ensures that the date of assessment is specified in the Admission slip.

· Informs candidate of requirements and preparation for the application, admission and schedule of assessment.

   

Data Encoder

· Encode all the necessary information to TESDA system or T2MIS.

Cashier

· Ensures that the Assessment Fee is paid only to the authorized person in the Assessment Center and issue Official Receipt issued to the applicant.


II. APPLICATION FOR COMPETENCY ASSESSMENT (Per Qualification) 

  • Submit Letter of Intent to the TESDA Provincial Office

  • Ready and prepare the necessary Corporate Documents based on the checklist of requirements for Competency Assessment Center provided by TESDA:

- Copy of SEC Registration or equivalent (CDA-registered, R.A., except Sole Proprietorship)

- Financial Statement (Latest Audited)

- Business Permit (Current and Valid)

- Fire Safety Certificate (Current and Valid)

- BIR Registration (Valid)

- Company Profile

- Organizational Structure

- Staff Complement and Profile

- Building layout/floor plan/shop lay-out

- Self-Assessment Checklist (TESDA-OP-CO-03-F03)

- List of complete facilities, tools, equipment, and materials appropriate to the qualification/applied for (identified in the CATs)

- Location Map

- Lease Contract/Proof of Ownership of the location /premises of the Assessment Center

- Checklist of tools, equipment, supplies and materials, and facilities (TESDA-OP-OC-03-F04

  • Wait for the Letter of Notice of Inspection for the said AC.

  • The team members’ representative from TESDA Regional Office together with Provincial Office and Technical Expert are the authorized to conduct the inspection.

  • Wait for the Letter of Notice for the result of conducted inspection.

  • Upon receiving of the Letter of Notice of Inspection Result, pay the necessary fee for Accreditation of Assessment Center at TESDA Provincial Office together with the Affidavit of Undertaking

  • Receive the Accreditation Certificate from TESDA Provincial Office.


III. APPLICATION FOR COMPETENCY ASSESSMENT CENTER

IV. CONDUCT COMPETENCY ASSESSMENT

 

TESDA Representative

  • Check the list of candidates scheduled for assessment.

  • Request for Assessment Package (TESDA-SOP-CO-07-025).

  • Prepares Assessment Packages

    - One copy of Assessor’s Guide

    - Specific Instruction to the Candidate, CARS, and Rating Sheets corresponding to the number of candidates to be assessed.

  • Arrives at the Assessment Center/Venue ahead of schedule to ensure that the assessment starts on time.

  • Issues Assessment Packages to the Competency Assessor.

  • Checks the Attendance Sheet vis-à-vis the actual number of candidatess

  • Ensures that the assessment is conducted in accordance with the requirements as identified in the Competency Asssessment Tools (CATs).

  • Checks quantity and quality or returned Assessment Packages, including the Attendance Sheet and informs Competency Assessor of any discrepancies.

  • Prepares Report on Assessment Proceedings (TESDA-SOP-CO-07-F27).

  • Receives complaints of clients (if any).

  • Receives assessment documents from the AC Manager.

- Accomplish Attendance Sheet

- Accomplished Rating Sheets

  • Requests identified respondents to accomplish the Competency Assessor’s Performance Evaluation Instrument (TESDA-SOP-CO-F19) at the end of assessment.

  • Signs the Attendance Sheet (after review/validation of the result of assessment from the Rating Sheets and CARS)

  • Returns assessment documents to PO at the end of the Assessment activity.
