  • 1. Proof of Entrance Examination Result given by the institution.

    2. Original Copy of Form 137 (SF10) Student Permanent Record and Form 138 High School Card (SF9).

    3. Original Copy of Certificate of Good Moral Character.

    4. Original Copy of PSA/NSO Birth Certificate.

    5. Four (4) pcs. 1"x"1 ID picture with white background.

    6. Medical Certificate.

    For Maritime Programs: (Physical Exam, X-ray, Hearing Test, Psychiatric Test/Neuro, Ishihara/Colorblindness Test).

    For Non-Maritime Programs: (Physical exam, X-ray, CBC, Urinalysis, Stool Exam, HEPA Screening).

    7. Vaccination Card.

    8. PhilHealth Member Data Record (MDR).

    3. Original copy of Honorable Dismissal / Certificate of Eligibility to Transfer.
