Senior High School and College Admission Requirements

 

For Senior High School:

  • 1. Original Copy of Grade 10 Report Card (SF9).

    2. Original Copy of Certificate of Good Moral Character.

    3. Original copy of PSA/NSO Birth Certificate.

    4. Four (4) pcs. 1"x"1 ID picture with white background.

    5. Qualified Voucher Recipient(QVR) Certificate or ESC Certificate.

  • 1. ALS Certificate / Diploma.

    2. Original copy of PSA/NSO Birth Certificate.

    3. Four (4) pcs. 1"x"1 ID picture with white background.

    4. Qualified Voucher Recipient(QVR) Certificate or ESC Certificate.

 

For College:

  • 1. Proof of Entrance Examination Result given by the institution.

    2. Original Copy of Form 137 (SF10) Student Permanent Record and Form 138 High School Card (SF9).

    3. Original Copy of Certificate of Good Moral Character.

    4. Original Copy of PSA/NSO Birth Certificate.

    5. Four (4) pcs. 1"x"1 ID picture with white background.

    6. Medical Certificate.

    For Maritime Programs: (Physical Exam, X-ray, Hearing Test, Psychiatric Test/Neuro, Ishihara/Colorblindness Test).

    For Non-Maritime Programs: (Physical exam, X-ray, CBC, Urinalysis, Stool Exam, HEPA Screening).

    7. Vaccination Card.

    8. PhilHealth Member Data Record (MDR).

  • 1. Proof of Entrance Examination Result given by the institution.

    2. Original copy of True Copy of Grades (from previous school).

    3. Original copy of Honorable Dismissal / Certificate of Eligibility to Transfer.

    4. Original copy of Certificate of Good Moral Character.

    5. Original Copy of PSA/NSO Birth Certificate.

    6. Four (4) pcs. 1"x"1 ID picture with white background.

    7. Medical Certificate.

Hard copy of the listed requirements have to be submitted upon issuance of official receipt of tuition fee payment.
